Meal Preparation

Prepare modified diets ordered by a licensed health professional to accompany medications.

Direct Care

Assistance with dressing, self-feeding oral hygiene, hair care, grooming, toileting, and bathing

Our team typically review and respond within 1 - 2 business days with an initial admission determination based on the comprehensive review of paperwork received from the case manager/referral agency.
Pre-Admission Meeting
Based on the assessed level of care services, our team schedules a face2face meeting to ensure everyone makes an informed decision to move in to Gentle Haven.
Move In
Once a consensus has been reached, our admission team work with you and your case manager (where applicable) to schedule the move in date and a face-to-face intake assessment upon admission.
